Do you accept HSA/FSA payments?

Yes, we accept HSA and FSA payments. Please contact us if you have questions or need help.

Do you accept Medicaid or Medicare?

Sadly, we are not able to accept Medicaid or Medicare at this time.

What insurances are you in network with?

We are in network with most United Healthcare/UHC plans, and most UMR plans, but benefits will vary according to your individual plan. We also offer flexible payment options including HSA/FSA.

Our Anthem BCBS patient panel is currently full.

Options for care:

1. Join our Anthem BCBS waitlist

2. Begin care at our self-pay rates:

• Initial Visit: $150

• Follow-up Visits: $100

If you start as a self-pay patient and an Anthem spot becomes available, we can transition your care to insurance coverage.
